Add a form that allowers users to email the instance owner

... and apply this in the missing contact details page.

This is a very simple implementation with some known limitations:

 * No verification is done of the email address (e.g. "click on the link
   to send") the user enters as their own.

 * There's no captcha to stop spambots using it to send email to the
   instance owner.
