Drop the async dependency and finish the async/await conversion

Convert the remaining Promise-constructor executors (set, del, bulkSet,
mutex.lock, mutex.unlock) to linear async functions over promisified
client calls, replacing mutex.lock's async.retry with the same retry
loop semaphore.acquireLock uses. With the test suite's async.each and
async.times usages converted to Promise.all, nothing depends on the
async package anymore, so it's removed from dependencies. Also drop
fetchAndRefresh's vestigial _this alias.
