docs: show how to map the refresh token entity yourself

The shipped entity is a mapped superclass declaring the identifier and its AUTO
strategy, which a subclass cannot override: Doctrine refuses an identifier
declared twice, and AttributeOverride only reaches a column definition, not the
generation strategy. On PostgreSQL, Doctrine asks for that strategy to be chosen
explicitly, so there was no way through without extending the model instead.

That path was only ever described in an issue, so it is now documented and held
to the shared contract suite by an entity mapping itself with an explicit
strategy.
