Version 4.0.0: full async/await support, callbacks deprecated

Bump to 4.0.0 to signal that every function now supports async/await;
callback support is deprecated and will be removed in v5. Close the
last gap by letting bulkFetch's cache-miss function be async — the
executeFunc helper now takes leading arguments and detects a callback
by comparing declared parameters against them.
