docs: Say what -opaque-errors covers

It withholds why a request was refused, not whether its document is on
the list — a forwarded document comes back with the API's own answer, so
nothing hides that. An upstream failure keeps its 502 or 504 rather than
collapsing into the 403. TestOpaqueErrors pins both against one server.
