docs: prepare the 2.1.0 release

findOneBy() is described with a @method tag rather than declared on
RefreshTokenRepositoryInterface, so a repository implementing it keeps
the inherited signature and nothing breaks. The bundle still calls it
with an order, which is what the tag documents.

The changelog section named Unreleased was what shipped as 2.0.0, so it
is renamed, and 2.1.0 is written above it. UPGRADE-2.1.md covers the
return values an application may be reading and the values now rejected,
and the readme points at both guides.
