fix: Remove Advanced group headings and rebalance option spacing

The Advanced options panel dropped its four group captions (BASIC OPTIONS,
SPEED & SILENCE, OUTPUT, PROCESSING & APPEARANCE): every row is already
labelled, so the headings only spent vertical space. Row spacing carries the
grouping instead — a shared SETTING_ROW_PADY above each option row (replacing
the per-row mix of (8, 0) and 4), and a wider PRESET_ROW_PADY around the Preset
strip so it reads as its own band.

Drops the now-unused add_group_heading helper and the Heading.TLabel style.
