chore(claude): require 100% coverage in the api-stability audit

The audit checklist ran the coverage command without saying what it had
to show. Spell out that every way of extending a function — an overload,
an optional parameter, a newly accepted input type — adds branches that
each need their own test, and that the threshold in vitest.config.ts is
never the thing that moves.
