Pin the dead-caller semantics of the coverage fixpoint (#842)

An eager call site inside a function that is itself unreachable
(defined, never referenced) contributes no executed path and no
call-graph node, so it must not break caller coverage. The corpus
evidence on the phase-2 issue surfaced exactly this shape: an eval-only
image loader left dead in the subject calling an otherwise-covered
function. Make the choice a fixture rather than an accident before any
blocking behavior ships on the advisory.
