chore(claude): add CLAUDE.md and Claude Code skills

Give Claude Code the repository's non-negotiable rules up front — no
breaking change ever, zero runtime dependencies, 100% coverage — plus
four skills covering the documentation-then-test workflow for a new
function, auditing a change for API breakage, killing surviving Stryker
mutants, and the conventional-commit format semantic-release depends on.
Until now each session had to rediscover all of it from the sources and
the git history.
