fix: prevent Report notice lifetime misuse (DAL-57) (#273)

* Fix Report notice value lifetime

Materialize the header label count for exception stack diagnostics so the registered address remains valid until AddHeaderRow exits. Strengthen the wrong-width regression test to verify the diagnostic value.

* Complete Report width diagnostics

Reuse materialized width counts for validation, error formatting, and resize so the expected width has a single source. Assert the exact business message while preserving the nLabels lifetime regression.
