feat: PE-09 credential record model (#149)

* feat: add credential record entity (PE-09)

Persist issued-credential metadata with FKs to tenant, connection,
operation, issuance_profile, and connector_credential. Migration
000011 follows PE-10 profiles. HTTP list/detail deferred until
auth/scopes land.

* fix: require credential.connector_id per PE-09

Match #32: connector_id is NOT NULL with ON DELETE RESTRICT so
revocation routing always has an adapter target.
