Drop callback support and release 1.0.0

Every caller now awaits: bloodhound at carriers/dhlEcommerceSolutions.js
and fulfillment-service at all five of its DHL call sites. The callback
branch had no users left, so each method becomes a plain async function
and the executor wrapper goes away with it.

The callback tests were converted to await rather than deleted. Only the
success and rejection pairs added in #11 had await twins; the rest cover
behaviour nothing else asserts, including non-200 handling, a missing
request body, PNG label format and access token reuse. Where a converted
test matched an existing twin exactly the duplicate was dropped, and the
twins kept the `err.status` assertion the callback versions carried.

index.js goes from 331 lines to 233, the suite from 1617 to 1047.
