Add a CHANGELOG, backfilled from git history and npm publish times

The repo had no changelog. Release notes lived only in GitHub Releases, where the single
existing release (v0.4.0) carries GitHub's auto-generated "What's Changed" list of PR titles.
That mechanism reads PR titles, so it cannot describe the five breaking changes in 0.5.0.

Backfilled 0.0.1 through 0.4.0. Each version's contents were attributed by comparing commit
timestamps against the npm publish time for that version rather than against the package.json
bump commit, which matters twice: the `downloadManifest(pickup, requestId, callback)` signature
change landed nine minutes before 0.1.0 was published, so it belongs to 0.1.0, not 0.2.0.

Two things worth stating in the file rather than leaving as puzzles: only v0.4.0 has a git tag,
so older releases have no comparison links; and 0.3.1 was bumped in package.json but never
published to npm, so it gets no entry.

0.4.0 is recorded as having no runtime changes because it has none — `git diff afaa84d 6454f62
-- index.js` is empty. It was a license and tooling release.

The 0.5.0 heading says "Unreleased" and needs a date when the tag is cut.
