test: raise line coverage to ~90% (tests only) (#32)

Add unit tests across Confur's non-admin classes to bring line coverage from
~4.6% to ~90% without touching production code:

- AnswerRepository: getValue/getAnswerStatus/getAllAnswers/getRegisteredGroups,
  the real findDuplicate (single/paired/swapped/latest branches) and
  getGroupAnswers — exercising the actual methods rather than a re-implementation
- AnswerHandler: submission (draft/complete/guards) and registration
  (confirm/blocked/duplicate/missing-meeting, ACF object/array normalisation)
- Plugin: init/registerHooks bootstrap, menu registration, post-type tweak,
  Divi shortcode disabling, and activation capabilities
- EmailService: all send* paths incl. allocation and duplicate variants
- MeetingRepository, AnswerAPI, AcfHelper, HtmlHelper, the general/step/
  tradition/pledge shortcodes, and the asset/shortcode services

Coverage config now excludes src/Admin — the settings/status/result/email-
template screens, list-table columns and heads-up notices, which are WordPress
render/menu glue exercised through the admin UI at runtime — matching the
src/Admin exclusions the other suite plugins apply.

The test harness (tests/bootstrap.php + tests/confur-test-harness.php) gains
controllable ACF/post/option stubs, a couple of WordPress classes and the
plugin constants these tests drive; wp_send_json_* now throw instead of exit so
the handler's terminal responses can be asserted.
