test: raise line coverage to ~95% (tests only) (#21)

Add unit tests across the LifeLines Lookup subsystem to bring line coverage
from ~2.5% to ~95% without touching production code:

- LookupSettings: stored-option reads with whitelist fallbacks, clamped
  accessors, and the sanitising save() incl. its empty-config guard
- TownRepository: search guards and the prepared-query happy path
- LookupController: hook/asset registration, shortcode HTML render, and the
  AJAX short-term and search branches
- TownSchema: tableName/exists/count/install, the import() parser (header,
  auto-id, numeric/blank NULL, batch flush, batch error, guards) and the
  exportCsv streaming loop
- LookupBootstrap: front-end vs admin registration and activation page-creation

Coverage config excludes src/Lookup/SettingsPage.php — the admin settings
screen (WordPress menu/enqueue hooks, form render, file-upload handling
exercised through WordPress at runtime), matching the src/Admin exclusions in
the other suite plugins. tests/bootstrap.php gains the small WordPress surface
these tests drive, plus a FakeWpdb stub.
