Add Pulse for coalescing on-demand work (#8)

* Add Pulse for coalescing on-demand work.

Introduce async.Pulse for event-driven background loops with optional
periodic repair, and simplify Repeat/Pulse to take func(context.Context)
since loop results were never used.
