Add cover.sh helper for scoped per-subject coverage

.claude/scripts/cover.sh <subject> runs the mirrored test under nyc scoped to
lib/<subject>.js (invoking node_modules/.bin/nyc and mocha directly) and prints
its uncovered lines, for fast iteration without the full npm run coverage. It
wraps the two footguns of a hand-rolled scoped run: --no-all (report only the
included file) and --globals __coverage__ (so mocha's --check-leaks does not
flag nyc's global and fail a passing test). Documented in test/CLAUDE.md.
