Publish Decision Guides natively in the docs (supersedes #9371) (#9927)

* docs: publish Decision Guides natively instead of linking to Google Docs

Supersedes #9371. Moves the seven Discovery & Decision Guides into
packages/docs/docs/decision-guides/*.md as versioned, Docusaurus-rendered
pages instead of hand-maintained Google Docs, so the guides can't drift
from the product. Rewires the hub page, workflow-index :::tip callouts,
the home.mdx step, and sidebars.ts to point at the new internal routes,
and adds the two new guides (RCM/Billing, E-Prescribe) that weren't part
of the original PR.

* docs: replace raw 'For Reference: URL' lines with a companion-link sentence

The intro line under each guide's H1 read the bare URL as its own link
text ("For Reference: https://www.medplum.com/docs/intake"), which is
clunky next to the natural prose everywhere else in the guides. Replaced
with a single italic sentence naming the docs page instead.

Absolute (not relative) URLs are kept deliberately: this same Markdown
also feeds the docx/PDF downloads (packages/docs/scripts/
build-decision-guides.mjs), and a relative /docs/... link wouldn't
resolve once someone opens a downloaded copy outside the site.

* docs: strip trailing hard-break whitespace from bullet lists

Every bullet item but the last in a list (across 6 of the 7 guides)
ended with a trailing "  " — a markdown hard line break — left over
from whatever tool originally exported these from Google Docs. Both
pandoc and Docusaurus/CommonMark render that as an explicit line break,
so nearly every bullet had extra vertical space after it: visibly wider
gaps between list items than between anything else on the page, in
both the rendered docs page and the docx/PDF downloads.
