* docs: delete five stale planning/research docs and the index that tiered them

docs/PLAN.md, docs/research/{PR-75-REVIEW.md,idea1.txt},
docs/design/TESTING_SCENARIOS.md, docs/IMPROVEMENTS_FROM_HEROKU_VALIDATION.md
and docs/INDEX.md were pre-implementation artefacts, and their rot is already
realized: PLAN.md's settings block omits five shipped knobs while CLAUDE.md
told engine-changers to read it first, and TESTING_SCENARIOS.md is linked from
the README as "Full design" while naming functions that do not exist. 2,311
lines, none of it packaged or read by a test.

Repoint every inbound link (CLAUDE.md, README.md, testing/__init__.py,
BACKGROUND_TRANSITION_ANALYSIS.md), and carry the two still-open items out of
IMPROVEMENTS_FROM_HEROKU_VALIDATION.md into TODO.md before deleting it.
docs/research/race-condition-issue stays — two stability tests cite it as
their reproduction source.

Also: de-duplicate the prose that README and CHANGELOG already own
(_release_lock's DEFER trade-off list was triplicated and the docstring itself
said "see README"); collapse the README testing chapter's overlap with
docs/TESTING_GUIDE.md, which is its live superset; drop TODO.md's 0.3.0-COMPLETE
block, a parallel changelog of 15 released items; and delete
test_readme_has_no_stale_release_instructions, whose sentinel 'legacy 0.1.x'
never appeared in any README commit — it was green while the drift it targeted
("0.9.0 at the time of writing", at 0.9.1) was live.

Write the comment/docstring rule into CLAUDE.md. Three reviewers recently
cited it as an existing rule; it was not.
