chore: update default codex model to gpt-5.6-sol and effort to high (#421)

* test(status): cover section constructors

NewTaskIterationSection, NewCodexIterationSection, NewClaudeEvalSection,
NewGenericSection, NewPlanIterationSection, and NewCustomIterationSection had
no tests. Table test asserts the Type/Iteration/Label contract for each,
taking pkg/status from 71.4% to 100%.

* chore: update default codex model to gpt-5.6-sol

set codex_model to gpt-5.6-sol and codex_reasoning_effort to high in the
embedded config. Effort drops a tier from xhigh, so external codex review in
default-claude mode runs at high unless codex_reasoning_effort is set
explicitly.

the effort-only doc example moves from --task-model=:high to :low. With high
as the new default, :high no longer demonstrates an override.
