docs: ask for one commit per concern, and route agents to CONTRIBUTING.md (#1295)

* docs(contributing): ask for one commit per concern

Bundled commits make review slow. Re-cutting a branch into single-purpose
commits also works as a review pass in its own right, which matters most for
agent-authored branches.

* docs(agents): point agents at CONTRIBUTING.md for the PR workflow

AGENTS.md described the toolchain but never said how changes are supposed to
land, so agents had no reason to read CONTRIBUTING.md.
