Extend AGENTS.md comment rule to future-phase notes

Fold the "no future/provisional state" and "no ADR citation" rules
into a single section instead of two overlapping ones, so there's
one place a contributor (human or AI) checks instead of reconciling
near-duplicate wording. Also add an explicit escalation path: an
urge to write a forward-looking or rationale-restating comment
should be treated as a signal that an ADR or the implementation
plan is under-specified, not papered over with a comment.
