Fix BCG tolerance-converged initial guesses (#261)

Return before the first bi-conjugate update when the supplied guess already meets the requested residual tolerance. This avoids a zero-denominator breakdown on otherwise acceptable nonsymmetric systems while preserving CG refinement semantics and exact-underflow handling.
