Load fwtoolkit color variables on admin pages

Admin templates that load the app-specific css/colors.css override were
missing the base css/fwtoolkit/colors.css where --cs-* variables such as
--cs-white-text are defined. Add the base color layer to:

- base/templates/admin/console.html
- document/templates/admin/document/maintenance.html
- document/templates/admin/document/documenttemplate/change_form.html

Also load document_template_admin.css on the document template change_form
page; it contains admin-only overrides used by the template editor.
