Move ctgrind ABI adapters into ct-fixtures (#182)

The `ctgrind_*` shape macros (bin/un/pred/pred2/count/shift/checked_*/
cond_select/carrying_add/asym_*) describe ct-fixtures' own fixture ABI, not
anything shared — they're named after representative methods but carry zero
method logic, just an `extern "C"` signature plus taint/untaint of its
arguments. Keeping them in the harness crate meant a new calling shape (e.g.
a widening `carrying_mul` with two outputs) couldn't be added without a
harness release.
