fix: reconcile NULL title default between openapi tree and stream emitters (#1062)

- bb_openapi_emit() now defaults a NULL meta->title to "breadboard device", matching bb_openapi_emit_stream() (the device-serving path is unchanged)
- add NULL-title and NULL-version fixtures to the equivalence harness, asserting both emitters agree
- update the harness header comment: the title divergence is now reconciled, not a tracked follow-up
- update test_openapi_emit_null_title_defaults_to_empty (now ..._to_breadboard_device) to assert the new shared default
