fix: safely handle errors from EffectiveTLDPlusOne

Ignore errors when deriving eTLD+1 for apex domain checks since bare
TLDs and invalid labels naturally fail public suffix extraction. Also
skip the apex lookup entirely if the subdomain is already flagged as
blocked by the bloom filter.

Fixes #226
