refactor: replace custom: name prefix with commitment provenance tagging

User-given commitment names are now kept exactly as typed. Commitments
carry a provenance field (scheduler|custom); cost reporting keys stay
the user's names, summing same-named same-provenance entries (per-device
copies previously overwrote each other), and only on a name collision
with a scheduler-internal commitment is the custom entry reported under
a '<name> (custom)' suffix, with a warning. This supersedes PR #2285's
custom: prefix (folded into this PR, changelog entry dropped).
