iter: don't yield a phantom (nil, nil) after MsgsTimeout's ErrTimeout (#2093)

After NextMsg returns ErrTimeout the iterator was yielding (nil, err),
honoring the caller's 'keep going' signal correctly, and then falling
through to yield(msg, nil) with msg still nil. So a 'for msg, err :=
range sub.MsgsTimeout(...)' that continued past a timeout was getting
two yields per tick: one (nil, ErrTimeout) and then a phantom (nil, nil).

Mirrors the rest of the function. Added a regression test that asserts
three consecutive yields against a quiet subject are all ErrTimeout
with no phantoms in between; fails on master.
