Defer completed-row commit to Close after append failure

When a row callback or EndRow fails, the appender no longer
flushes and closes itself immediately. The failed row is discarded,
the rows completed before the failure stay buffered, and Close is
now allowed on a faulted appender so those rows are committed
there. The callback exception propagates on its own instead of
being wrapped in an AggregateException; errors from committing the
completed rows (e.g. constraint violations) surface at Close.
