Fix ListConfig insertion and union flag invalidation

Normalize ListConfig insertion indices with Python's integer-index protocol and list insertion rules. Use _set_item_impl() for wrapping and validation, and roll back atomically on failure.

Propagate inherited flag-cache invalidation through a UnionNode to its selected child.

Add regression coverage and news fragments for both fixes.
