style: rephrase flattened em-dashes as real punctuation

88 fixes across doc comments, code comments, and user-facing status/error
message strings (rename/replace/gist/pull messages): ` - ` replaced with
commas, colons, semicolons, or parentheticals; assembled status lines now
comma-join their clauses. Left untouched: arithmetic and the `render -`
stdin argument (the CLI-flag doc now shows it as `\`-\``). 278 tests pass.
