Add v2 planning convergence telemetry (#180)

* Add v2 planning convergence telemetry

Expose completed-pass convergence summaries, blocker attribution, and pass-to-pass fingerprints for the streaming v2 indexer. Extend directional-PCA telemetry with deterministic planner-state fingerprints and progress detail, and cover the new telemetry in the streaming-indexer spec and validation tests.

Handle immediate repeated-pass detection, rename the previous-pass field for clarity, and replace the retained multi-pass full snapshot history with a compact repeat-history plus only the latest detailed snapshot.

Make blocker-detail strings caller-friendly by rendering unknown counts explicitly, and stop phase-only RealizePartition telemetry from claiming realized-cell counts before realization state exists.

Tighten convergence-state classification so shrinkage is only reported when unresolved work truly shrinks, and cache directional-PCA telemetry snapshots to avoid recomputing expensive planner-state fingerprints on every observer read.

Keep unknown replay progress explicitly unknown in blocker attribution and move directional PCA fingerprints back to raw bytes so trainer telemetry stays Copy and cheap to emit.
