docs: add module and symbol docs for cli and node-runtime

Add @module JSDoc to all JSR entrypoints in @forst/cli and
@forst/node-runtime. Document exported symbols across node-runtime so
generated docs and the symbol-documentation score meet JSR requirements.

fix(node-runtime): satisfy JSR slow-types validation

Add explicit public API type annotations, register.d.ts with
@ts-self-types for register.mjs, node:url URL imports, and TaggedError
superclass extraction. Re-export internal types referenced by public
signatures and ship register.d.ts in jsr publish and dist build output.
