fix: restore scope-creep check, dedupe closing line (review notes)

- implementation.txt gains a scope-creep responsibility - the old
  quality.txt "changes solve only the stated problem" bullet was
  dropped with the Simplicity Assessment section and had no owner left
- the "Report findings only - no positive observations" closing line
  is removed from the five agent bodies; formatAgentExpansion already
  appends it once, so the composed prompt carried it twice
