perf(parser): value-type Action and flat (state,symbol) tables

Replace the virtual Action hierarchy and nested unique_ptr maps with:

- Value-type Action cells (shift/reduce/accept/error factories)
- Flat unordered_map<StateSymbolKey, Action> for lookahead actions
- Sparse per-state error candidates instead of dense error cells
- Flat gotoTable keyed by StateSymbolKey

GeneratedParsingTable uses Action factories and bitset reduce emission;
unit tests cover value Action and sparse LookaheadActionTable.
