refactor: improve blocklist comment handling and testing

- Skip single-line comments in `scanBlocklist` to ensure accuracy.
- Update `countFromFile` logic to ignore comments, fixing an off-by-four
  error in domain count reporting.
- Refactor test suite to use a shared `setupTempFile` helper for better
  maintainability and cleanup consistency.
