fix(examples,docs): supply organization context for login/refresh (CONTRACT §5.1) (#8)

The login and refresh endpoints require ORGANIZATION context in addition to
tenant context — a tenant slug is only unique within an organization, so the
server rejects a login body without an org identifier (HTTP 400 "must provide
org_id or org_slug"). Update the docs and the runnable login example to pass
org context, and re-sync the vendored CONTRACT.md from the upstream repo.

- CONTRACT.md: re-synced from axiam/sdks/CONTRACT.md (adds §5.1).
- README.md: Quickstart login snippet now passes orgSlug.
- examples/login_mfa.php: constructs the client with orgSlug (reads
  AXIAM_ORG_SLUG, default "acme"); it is the only example that calls
  login()/refresh().

Resource-server examples (Laravel/Symfony bridges, rest_authz.php,
grpc_checkaccess.php) only verify tokens or run authz checks and never call
login/refresh, so no org context was added there. SDK public API is unchanged.
