fix(examples,docs): supply organization context for login/refresh (CONTRACT §5.1) (#11)

Login and refresh require organization context (org_slug/org_id) in
addition to tenant context — a tenant slug is only unique within an
organization, and the server rejects a login body without an org
identifier with 400 "must provide org_id or org_slug" (CONTRACT §5.1).

- Re-sync CONTRACT.md from the main repo (adds §5.1).
- login-mfa, authz-check, grpc-checkaccess examples: construct the client
  with axiam.WithOrgSlug, reading AXIAM_ORG_SLUG (default "acme"),
  mirroring the existing AXIAM_TENANT_SLUG usage.
- README §5 login snippet: pass WithOrgSlug.

Resource-server / route-guard code that only verifies incoming tokens or
calls CheckAccessAs (middleware-guard example, README middleware and
RequireAccess snippets) and the mTLS device-auth snippet are intentionally
left unchanged — they never call login/refresh, so org context does not
apply (CONTRACT §5.1).
