fix(examples,docs): supply organization context for login/refresh (CONTRACT §5.1) (#12)

The login and refresh endpoints require organization context in addition
to tenant context (a tenant slug is only unique within an organization).
Re-sync the vendored CONTRACT.md and update every example, README/doc
snippet that constructs a client and calls login/refresh to supply
org context (org_slug, defaulting to "acme" via AXIAM_ORG_SLUG in runnable
examples). Verify-only examples (fastapi/django middleware, require_access
authz clients, mTLS snippet) are intentionally left unchanged — they never
call login/refresh and §5.1 makes org context optional for that usage.
