fix(examples,docs): supply organization context for login/refresh (CONTRACT §5.1) (#18)

Login and refresh require organization context in addition to the tenant
(a tenant slug is only unique within an organization). Update the examples
and README/doc snippets that construct a client which actually performs
login/refresh so they pass orgSlug, and re-sync the vendored CONTRACT.md
from the main repo (adds §5.1).

Resource-server examples (express/nestjs/fastify apps and the middleware
README snippets) only verify incoming tokens and never call login/refresh,
so they are intentionally left unchanged.
