Resolve per-LLM timeout question; add wait-bounds-attempt contract fix

Reject a per-LLM HTTP timeout permanently: latency budget belongs to the
call, not the model, and does not compose with failover. Instead make
`wait` bound the in-flight attempt (per-attempt timeout = min(remaining,
_HTTP_TIMEOUT)), distinguishing budget exhaustion (do not cool the model)
from a genuinely slow model (cool down + fail over). Records the finding
and folds the fix into Phase 2; adds a Settled non-goals section.
