add A13+A14 probes + ledger run 12

- hot_upgrade/ probe (CI-isolated, teeth-proven): failed reload
  leaves old code and live resources running, no VM abort; rustler
  0.38 hardcodes the NIF entry upgrade callback to NULL
- guides/gotchas.md: hot code upgrades unsupported — restart the
  node; the failure mode is loud and safe
- test_arch/ probe: ~830k parallel isolated-DB ops, zero corruption
  or NOMEM; mechanism graded plausible, test.seq stays load-bearing;
  precompiled artifacts bundle SQLite statically with no exported
  sqlite3 symbols, so the per-process-globals reasoning holds
- CLAUDE.md gotcha #1 reworded: shared-globals contention under
  THREADSAFE=1 mutexes, not memory corruption
- BACKLOG: F-A13-1 (upstream rustler upgrade gap, tracking) and
  F-A14-1 (constrained-RAM deciding probe, deferred) filed as S3
