Untrack CLAUDE.md; add it to .gitignore

CLAUDE.md is the project-local working-notes file (it says so in its own
header) and was never meant to ship in the public repo. It was never
actually covered by any ignore rule, so it got swept in by an over-broad
`git add`. Remove it from tracking and add the ignore entry so it can't
recur.
