test: exclude compile-time macro from coverage (#28)

Attio.Paginated.__using__/1 runs at compile time and generates stream/2 and
stream_all/2 under the caller modules (Notes/Tasks/Meetings/Threads), so cover
attributes those hits to the callers, never to this file. The generated code is
already exercised by each caller's stream tests. Wrap the macro body in
coveralls-ignore markers so the misattributed lines stop dragging total coverage
below 100%.
