Remove completed adapters-subpackage plan; note web auto-provisioning

Both phases (adapters/receipts/ and adapters/rates/) are implemented, so
the plan file has served its purpose and is removed. The subpackage
grouping is an internal code-organization detail that specs deliberately
omit (architectural decisions and business requirements only), and
architecture.md already describes adapters/ at the right altitude — no
spec change is needed.

Also document, in AGENTS.md, that pointing the Claude Code web
environment's setup script at scripts/setup-test-env.sh gives every
session a pre-provisioned environment.
