* feat(plugins): add Codex automatic plan review via Stop hook

Add an opt-in Codex Stop hook while preserving Claude default hook discovery. Gate review on Plan mode, fall back to the exact session and turn transcript whenever the payload lacks a complete plan, and fail open when event data or runtime dependencies are unavailable.

Carry annotated revisions through guarded snapshots and rolling comparisons, with runtime-specific hook manifests, integration coverage, and matching launcher documentation.

* fix(revdiff-planning): harden hook test portability

Resolve the active Python interpreter through PATH before tests sandbox child environments, avoiding accidental fallback to platform Python through version-manager shims.

Build hook payloads and manifests from structured JSON, use portable temp paths and explicit expectations, and ignore invalid cwd payload types without breaking the fail-open hook path.

* fix(plugins): harden Codex plan review transcript selection

Prefer plan-message transcripts when available, keep snapshot creation and cleanup safe on interruption, and add a sanitized live rollout regression that proves exact-session fallback stays on the intended transcript.

* fix(plugins): ignore empty proposed_plan blocks in Codex review

The Codex plan review hook could silently no-op when the transcript ended with an empty <proposed_plan> block, or when a valid plan was followed by an empty closer. It also needed to keep the same transcript-message selection semantics while preferring the last non-empty complete plan.

Add regressions for empty-block closers, valid-plus-empty blocks, and trusted snapshots outside TMPDIR, and correct the marketplace path reference in CLAUDE.md.
