docs(rumqttc): add doc comments and improve diagnostics snapshot performance

Add comprehensive documentation to EventLoopDiagnostics, QueueDiagnostics,
SessionDiagnostics, OutboundDiagnostics, and their public fields across
both v4 and v5 crates. Document diagnostics() and outbound_diagnostics()
methods with usage examples and mark them #[must_use].

Optimize outbound_diagnostics() by pre-computing field values into locals
and inlining the outbound_drained check, replacing the previous O(n)
packet-identifier scan with the maintained outbound_pkid_count. Add tests
verifying packet-identifier count consistency across protocol transitions.
