add Xqlite.Telemetry.OpenTelemetry attribute mapping

a pure, dependency-free translation from xqlite telemetry events to
OpenTelemetry's STABLE database semantic conventions —
db.system.name, db.query.text, db.operation.name, db.namespace,
error.type — plus span_name/2 following the spec's priority chain.
Users call it from their own handler; database-aware backend
tooling keys off exactly these names. Every mapped name is cited to
its spec page in the module docs (verified against the stable
revision 2026-07-17; pre-stabilization names deliberately not
emitted). Wiring guide gains the section; the module doubles as the
attribute vocabulary should first-class instrumentation land later.
